We currently collect and process the following information:

2. Information pertaining to your physiotherapy treatment.

Most of the personal information we process is provided to us directly by you for one of the
following reasons:
1. To enable us to effectively and efficiently provide you with the service that you engaged
with us to provide.
2. To maintain our accounts and appropriate legally required records.
We also receive personal information indirectly, from the following sources in the following
scenarios:
1. GP practices and other healthcare professionals
2. Private rehabilitation companies on the instruction of your solicitors/insurers.
We use the information that you have given us in order to deliver the best treatment plan for you.
We may share this information with your GP and healthcare team, or the company who
referred you for treatment under directions from your solicitors / insurers.
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for
processing this information are:
(a) Your consent. You are able to remove your consent at any time. You can do this by

(b) We have a contractual obligation, with you, the NHS or the company referring you for
treatment.
(c) We have a legal obligation.

How we store your personal information:
Your information is securely stored at the clinic in which you were treated before being
archived at Head Office.
We keep your treatment notes for the designated length of time required by law. We will
then dispose your information by secure data disposal methods.
Your data protection rights:
Under data protection law, you have rights including:
a) Your right of access – You have the right to ask us for copies of your personal information.
b) Your right to rectification – You have the right to ask us to rectify personal information
you think is inaccurate. You also have the right to ask us to complete information you think
is incomplete.
c) Your right to erasure – You have the right to ask us to erase your personal information in
certain circumstances.
d) Your right to restriction of processing – You have the right to ask us to restrict the
processing of your personal information in certain circumstances.
e) Your right to object to processing – You have the the right to object to the processing of
your personal information in certain circumstances.
f) Your right to data portability – You have the right to ask that we transfer the personal
information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we
have one month to respond to you.
